Design of a Social Determinants of Health Screening Tool Using Health Information Exchange and Mobile Technology

Abstract: Incorporating social determinants of health (SDoH) screening into regular healthcare practice can be difficult and time consuming. For this project, a regional health information exchange (HIE) led an inter-agency consortium to develop a system for SDoH screening and referrals using automated ADT messages through the HIE with a consumer-facing mobile application that addressed many practice-level barriers to SDoH screening.

Describe the new knowledge and additional skills the participant will gain after attending your presentation.: By attending this session, audience members should be able to: define the challenges facing health care professionals who wish to implement social determinants of health screening (SDoH) and intervention in their current care delivery environments; describe a consumer informatics tool used to detect and address SDoH; and list four community-level systems interventions essential to implement the tool as a smartphone-based application.
