Description

Abstract: We leveraged electronic health record (EHR) data to develop a novel pharmacovigilance tool that identifies hospitalizations with adverse drug reaction (ADR)-associated ICD10 codes. Medication Safety Pharmacists review these hospitalizations to identify unreported ADRs. Over a 2-year period, 289 patients were identified as having a new ADR that required entry into the ADR tracking system in the EHR. These ADRs were associated with 576 hospitalization days, thus has potential to improve patient safety and reduce hospitalizations.

Describe the new knowledge and additional skills the participant will gain after attending your presentation.: The use of ICD10 codes to identify adverse drug effects that either caused or occurred during a hospitalization is an additional novel surveillance tool that can be utilized to improve pharmacovigilance reporting.
