Description

Abstract: Our goal was to increase the percentage of hospital admission medication reconciliation(AdmMedRec) completion to ≥ 95% in 12 months. The project initiated in April 2017 with Electronic Health Record(EHR) workflow redesign. Interventions included hospitalist led targeted training, simplified EHR build, activated Best Practice Advisory to fire if AdmMedRec was incomplete, and weekly report of specialty specific performance data. Hospital AdmMedRec completion rate increased from baseline of 73% to 95% within 7 months and has been sustained at 92%.

2.Identify common challenges to medication reconciliation and solutions we employed to overcome them.
3.Utilize EPIC reporting tools to promote MedRec compliance by promoting individual user, division, and institution accountability and transparency.
